Role: Hospital to Home Care Assistant
Location: Northampton, Daventry, Towcester and Brackley
Requirements – Must have a driver’s license and access to a car
Hours: 7am till 2pm and 2:30pm till 10pm, PAID BY THE SHIFT

We are adding to the team that provides care and support to individuals in their own homes that are discharged from hospital. These teams are vital in preventing hospital readmission and providing the immediate support whilst longer term care pathways are sourced.

Responsibilities:
✅ Assisting the Senior carer in their responsibilities
✅ Delivering personal care, assisting with medication and manual handling
✅ Preparing drinks/meals for the client if needed
✅ Promoting the clients independence and reablement goals
